Send a referral
Call the front desk or fax the referral to (509) 555-0104. We confirm that we have it and call the patient the same working day.
What helps us
The patient's name and phone, your contact details, the area of concern and any precautions you want us to follow.
What you get back
A written note after the first visit and when the plan of care changes, sent the way you prefer.
Patients who prefer Spanish
Tell the patient to ask for Ofelia Esquivel or Delaney Hodges, or tell us and we schedule them with one of them.
